I think italics are primarily for emphasis. But: A pretentious character might drop French words into their speech, which would be emphasized. In narration, it depends. My first manuscript involves a Hungarian-American woman visiting the country for the first time in decades, and struggling with vocabulary. I italicize some of those words, depending on how naturally they come to her.

Many! Especially those that invert other tropes. I can get lost on TVTropes deconstructing the plots of my favorite books, movies, games and the like. Anything "abandoned" with horrible secrets to discover is like candy to me.
